An opportunity for a Staff Pharmacist specializing in Medication Therapy Management (MTM) is available with a 13-week contract-to-hire assignment. This role supports specialty MTM services and is distinct from short-term remote surge coverage. The position allows for onsite or remote work but requires a valid Louisiana Pharmacist license and licensure in at least five additional states from a specified list.

Key Responsibilities:

Prepare and dispense medications per physician orders, following established protocols.

Interpret and accurately transcribe prescriptions into computerized patient medication profiles.

Maintain complete and precise pharmacy patient medication records.

Dispense pharmaceuticals and controlled substances responsibly, ensuring compliance with legal requirements.

Monitor patient drug therapies for contraindications, interactions, allergies, and appropriate dosing.

Identify and report adverse drug reactions promptly and accurately.

Provide clinical consultations, suggesting cost-effective therapeutic alternatives as needed.

Document clinical interventions comprehensively.

Participate in quality improvement and medication use review activities, including monitoring and inspections.

Supervise pharmacy support personnel, verify technician activities, and assist in performance evaluations.

Organize and prioritize workload to ensure timely pharmacy services with minimal supervision.

Conduct medication storage area inspections, replacing outdated stock.

Respond professionally to telephone and in-person inquiries.

Qualifications:

Current Louisiana Pharmacist license required.

Licensure in a minimum of five additional states selected from Alaska, Arkansas, California, District of Columbia, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming.

Minimum of 10 additional state licenses beyond Louisiana within 18 months in this role.

Prior retail or hospital pharmacy experience required.
