Provide expert ambulatory clinical pharmacy services as a credentialed member of a healthcare team dedicated to optimizing medication outcomes and enhancing patient care. This role involves practicing under Collaborative Practice Agreements (CPAs) to deliver Comprehensive Medication Management (CMM) recognized nationally, focusing on chronic disease management and specialty medication coordination.

Key Responsibilities:

  • Deliver ambulatory clinical pharmacy services including assessment, monitoring, and documentation of therapeutic plans under pharmacy CPA privileges.
  • Conduct patient visits for medication reconciliation, lab monitoring, counseling, and disease state management.
  • Collaborate with providers and care teams to implement evidence-based medication interventions aimed at quality, safety, and cost reduction.
  • Manage referrals with closed-loop communication to referring clinicians.
  • Coordinate prior authorizations, financial assistance, and benefit investigations to support patient access.
  • Oversee specialty medication therapy initiation, monitoring, and continuity.
  • Provide drug information and educational support to patients, families, and healthcare staff.
  • Precept pharmacy students, interns, and residents as assigned.
  • Participate in research, policy development, quality improvement, and committee work advancing ambulatory pharmacy practice.
  • Document interventions in electronic health records adhering to national ambulatory pharmacy standards.
  • Required Qualifications:

  • Active Delaware Pharmacist License in good standing.
  • Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory).
  • Minimum one year of clinical pharmacy experience including residency.
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
  • Preferred Qualifications:

  • Doctor of Pharmacy (PharmD) strongly preferred.
  • Completion of PGY1 or PGY2 ambulatory care residency a strong plus.
  • Experience with Collaborative Practice Agreements.
  • Specialty medication management expertise.
  • Background in CMM, chronic disease management, or population health pharmacy.
  • Preceptor experience and prior authorization knowledge.
