A contract opportunity is available for a California-licensed Pharmacist to participate remotely in a Medication Therapy Management (MTM) pilot program focused on Medicaid patients. The role supports a clinical pharmacy initiative aimed at enhancing patient care through direct counseling, medication review, and collaborative health outcomes improvement.

This position offers a flexible, part-time schedule with approximately 10-20 hours per week, exclusively during daytime hours to facilitate patient outreach and counseling. A guaranteed minimum of 8 paid hours per week ensures consistent compensation regardless of workload fluctuations. Candidates must reside in California and be actively enrolled as an ORP (Ordering, Referring, and Prescribing) provider within the state’s Medicaid program.

Responsibilities:

  • Conduct comprehensive MTM consultations tailored to individual patient needs
  • Provide patient counseling and clear medication education
  • Review medication profiles to identify and recommend optimization opportunities
  • Accurately document clinical interventions and patient encounters
  • Collaborate with program stakeholders to promote positive patient outcomes
  • Ensure compliance with applicable state and federal regulations governing pharmacy practice
  • Required Qualifications:

  • Active and unrestricted California Pharmacist license
  • Current residency within California
  • Prior MTM experience is mandatory
  • Enrollment as an ORP provider in California Medicaid (Medi-Cal) required
  • Excellent communication and patient counseling capabilities
  • Ability to work independently in a remote setting with flexible scheduling
  • This contract spans 3-6 months, starting as soon as possible, providing a unique chance to contribute to an innovative Medicaid-focused clinical pharmacy program from anywhere within California.
