Soliant Health is seeking an experienced Clinical Pharmacist for a contract-to-hire opportunity with a growing home infusion pharmacy in Wisconsin. This position requires a minimum six-month contract commitment before consideration for permanent employment. The ideal candidate will have recent sterile compounding and home infusion experience, strong clinical judgment, and the ability to work independently in a fast-paced environment.

Responsibilities

  • Verify IV medication orders for accuracy, safety, and appropriateness.
  • Review patient profiles and clinical information to support optimal therapy outcomes.
  • Perform clinical pharmacy duties and provide medication recommendations as needed.
  • Compound sterile preparations in the clean room when required.
  • Ensure compliance with USP standards, state regulations, and company policies.
  • Collaborate with nurses, physicians, and pharmacy staff regarding patient care plans.
  • Participate in on-call responsibilities, including every fourth weekend and one weekday evening per week.
  • Qualifications

  • Active and clear Wisconsin Pharmacist License.
  • Minimum of 2 years of recent sterile compounding experience.
  • Recent IV pharmacy experience required.
  • CPR+ EMR experience, preferred.
  • Minimum of 2 years of recent home infusion experience preferred.
  • Skills

  • Strong knowledge of sterile compounding and IV therapies.
  • Excellent clinical decision-making abilities.
  • Effective communication and collaboration skills.
  • Ability to work independently and manage multiple priorities.
  • Familiarity with pharmacy compliance and quality standards.
  • Experience

  • 2+ years of recent sterile compounding experience required.
  • Home infusion pharmacy experience strongly preferred.
  • Experience verifying IV orders and supporting infusion therapies.
  • Work Hours

  • Monday through Friday
  • 7:30 AM – 6:00 PM (8-hour shifts)
  • Full-time schedule
  • On-call every fourth weekend and one evening per week
