Join a specialized healthcare role focused on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. This contract position offers an opportunity to apply your clinical expertise by managing sterile compounding and tailoring medication regimens to ensure optimal therapy outcomes for patients.

Contract Duration: Approximately 8-12 weeks

Schedule:

  • Monday to Friday, 8:30 AM to 5:00 PM
  • Potential on-call rotation
  • Key Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions within sterile, ventilated environments following all state and federal regulations.
  • Assess laboratory values and calculate pharmacokinetic dosing, including vancomycin, and monitor Total Parenteral Nutrition (TPN) to adjust treatment plans effectively.
  • Deliver thorough patient and caregiver education on infusion pump usage, sterile techniques, and catheter maintenance in the home setting.
  • Collaborate with physicians, home health nurses, and interdisciplinary teams to coordinate care and reduce hospital readmissions.
  • Ensure compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
  • Manage pharmacy inventory, including refrigerated products, and coordinate medication shipping to patients.
  • Qualifications and Experience:

  • 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy with direct involvement in pharmacy IV room operations.
  • Experience with IVIg is a plus.
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
  • Current, active pharmacist licenses in Arizona and Texas, in good standing.
  • Experience working with CareTend system preferred.
