Clinical Pharmacist - Home Infusion / TPN Management

Orange, CA | 13-Week Contract-to-Hire 

RPh on the Go is partnering with a rapidly growing home infusion pharmacy to hire a highly clinical pharmacist with advanced experience managing patients receiving Total Parenteral Nutrition (TPN). This is a patient-focused, clinical role working closely with physicians, dietitians, nurses, and pharmacy staff to optimize nutrition support and infusion therapy outcomes.

This is not a production pharmacist position. We are seeking a pharmacist who has been directly involved in the clinical management of TPN patients and can confidently make therapy recommendations, review laboratory results, and collaborate with interdisciplinary care teams.

Schedule

  • Monday-Friday
  • 8:00 AM - 4:00 PM
  • 40 hours per week
  • Responsibilities

  • Manage and monitor patients receiving TPN therapy
  • Review laboratory values and make clinical recommendations related to nutrition support
  • Evaluate TPN formulations, electrolytes, micronutrients, and therapy adjustments
  • Collaborate with dietitians, physicians, nurses, and other healthcare professionals
  • Counsel patients regarding infusion therapies and medication management
  • Assist with sterile compounding oversight and USP <797> compliance
  • Serve as a clinical resource for pharmacy staff and healthcare providers
  • Support transitions of care for patients discharged from hospitals into home infusion services
  • Required Qualifications

  • Active California Pharmacist license
  • Willingness to obtain Mississippi and Alabama pharmacist licenses (or already licensed)
  • Minimum 2 years of recent clinical pharmacy experience
  • Recent sterile compounding experience
  • Strong knowledge of USP <797>
  • Experience working directly with TPN patients
  • Highly Preferred Experience

  • Cycling and tapering TPN schedules
  • Managing patients at risk for refeeding syndrome
  • Monitoring line-related complications and infection risks
  • Coordinating care with dietitians, nurses, physicians, and caregivers
  • Patient education for home infusion administration
