IV Sterile Compounding Clinical Pharmacist - Converse, Texas
A specialized opportunity is available for a Sterile Compounding Pharmacist to support home infusion therapy services. This 13-week contract role is based in Converse, TX, with a schedule from 12 PM to 8 PM.
Responsibilities include evaluating patients for eligibility for home-based infusion therapy and developing, monitoring, and adjusting individualized medication plans grounded in clinical assessments, laboratory data, and pharmacokinetic principles. The role also requires preparation, verification, and dispensing of sterile medications adhering to USP standards as well as state and federal regulations. You will educate patients, caregivers, and healthcare providers on medication therapies and infusion administration. Collaboration with healthcare teams to ensure continuity of care and monitoring of patient outcomes is essential. Additional duties involve managing infusion pumps and administration equipment, maintaining quality assurance, providing drug information, maintaining documentation, and supporting compliance and medication safety initiatives.
Qualifications for this position include a Doctor of Pharmacy (PharmD) or graduation from an accredited School of Pharmacy, along with an active and unrestricted Texas Pharmacist license. Candidates should have one or more of the following: at least one year of sterile IV compounding experience; experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM); completion of an ASHP-accredited Pharmacy Residency; completion of an Advanced Pharmacy Practice Experience (APPE); or equivalent pharmacy experience.
Strong clinical knowledge of home infusion pharmacy, the ability to interpret laboratory values, and apply pharmacokinetics are required. Excellent communication, patient counseling, and customer service skills are essential for success in this role.
