Overview:
We are seeking a retail experienced Staff Pharmacist for either full-time interim or a contract-to-hire opportunity outside of Bismark, ND and corresponding Clinical Suites. The ideal candidate will coordinate and manage clinical programs and initiatives, deliver medication therapy management services, and collaborate with prescribers and interdisciplinary care teams to optimize patient care.
In this role, you’ll:
- Coordinate and manage clinical programs such as Medication Therapy Management (MTM), Annual Wellness Visits, Remote Patient Monitoring, and Disease State Management
- Deliver MTM services including Comprehensive Medication Reviews (CMR) and Disease State Management to optimize medication therapy and improve clinical outcomes
- Utilize evidence-based practices and clinical guidelines to formulate treatment plans
- Conduct patient-centered adherence services to identify and overcome barriers to medication adherence
- Collaborate with prescribers and care teams to develop patient-specific therapy goals
- Engage in patient consultation activities and limited traditional dispensing functions
Qualified candidates will possess:
- B.S. Degree or Pharm.D. in Pharmacy from an accredited institution
- Current North Dakota pharmacist license
- Certificate of training for Immunizations
- Certification in CPR or BCLS
- PharmD
Physical demands:
This position may involve long periods of standing and requires the ability to lift and/or move up to 40 pounds. Specific vision abilities and manual dexterity are also necessary.
