This role is ideal for a licensed Pharmacist specializing in home infusion and sterile compounding, offering an opportunity to apply clinical expertise in patient-centered pharmacy care. The contract position requires working day shifts over a 13-week period and is located in Irving, TX.

Key Responsibilities:

  • Evaluate patients' eligibility for home infusion therapy and facilitate home-based care.
  • Develop, monitor, and adjust individualized medication plans based on clinical assessments, lab results, and pharmacokinetic data.
  • Prepare, verify, and dispense sterile medications following USP standards and regulatory guidelines.
  • Educate patients, caregivers, and healthcare professionals regarding medication therapy and proper infusion techniques.
  • Collaborate with healthcare teams to ensure continuity of care and effective monitoring of patient outcomes.
  • Manage infusion pumps and other medication administration equipment while maintaining safety and quality assurance.
  • Provide drug information, maintain detailed documentation, and support regulatory compliance and medication safety programs.
  • Qualifications & Desired Experience:

  • Doctor of Pharmacy (PharmD) degree or graduation from an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.
  • At least one of the following:
    • Minimum 1 year of sterile IV compounding experience,
    • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
    • Completion of an ASHP-accredited Pharmacy Residency,
    • Completion of an Advanced Pharmacy Practice Experience (APPE), or
    • Equivalent pharmacy experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Proficient in interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
