RPh on the go is searching for a Pharmacist. The ideal candidate will have a year or more experience working in a long term facility

Shift: M-F, day/evening rotation (8am - 5pm, 11am - 7pm), rotating every other weekend. 

 

Key Responsibilities:

  1. Medication Management:

  2. Review and verify medication orders for accuracy and appropriateness.
  3. Monitor residents’ medication regimens to ensure efficacy, safety, and adherence.
  4. Assess and manage medication-related issues and make necessary adjustments.
  5. Clinical Consultation:

  6. Collaborate with physicians, nurses, and other healthcare providers to optimize pharmacotherapy for residents.
  7. Provide recommendations on medication therapy, dosage adjustments, and alternative treatments.
  8. Participate in interdisciplinary care meetings and care plan discussions.
  9. Medication Orders:

  10. Ensure accurate preparation and dispensing of medications, including oral, topical, and injectable forms.
  11. Oversee medication administration practices to ensure compliance with prescribed therapy.
  12. Patient Safety:

  13. Monitor residents for adverse drug reactions and medication errors.
  14. Conduct medication therapy reviews to identify and prevent drug interactions and contraindications.
  15. Develop and implement strategies to enhance medication safety and efficacy.
  16. Medication Reconciliation:

  17. Perform medication reconciliation during transitions of care, such as admissions, discharges, and transfers.
  18. Ensure accurate and up-to-date medication lists for all residents.
  19. Education and Training:

  20. Educate healthcare staff and residents on proper medication use, administration techniques, and potential side effects.
  21. Provide training to new staff and ongoing education to existing staff regarding medication-related topics.
  22. Policy and Protocol Development:

  23. Contribute to the development and implementation of medication use policies and clinical guidelines.
  24. Ensure adherence to regulatory requirements and best practices in medication management.
  25. Quality Improvement:

  26. Participate in quality improvement initiatives to enhance medication management and resident care.
  27. Monitor and evaluate medication-related outcomes and implement changes to improve care quality.
  28. Inventory Management:

  29. Oversee medication inventory, including procurement, storage, and distribution.
  30. Ensure compliance with inventory control procedures and regulatory standards.
  31. Documentation and Reporting:

  32. Maintain accurate and detailed documentation of medication-related activities and interventions.
  33. Report any medication errors, adverse events, or safety concerns in accordance with facility policies.


Required

  • Pharmacy experience. 
  • LTC experience a plus
  • Texas pharmacy license.
