Job Description: We are looking for a skilled and dedicated Outpatient Pharmacist to join our team in Santa Clarita, CA. This is a temp-to-hire opportunity, with an initial temporary period of 12 to 24 months and the potential for permanent placement based on performance and business needs.
In this role, you will be responsible for dispensing medications, providing patient education, ensuring proper medication management, and collaborating with healthcare teams to ensure the best outcomes for our outpatient population.
Key Responsibilities:

Dispense medications and provide counseling to outpatient patients

Review and verify prescriptions for accuracy and appropriateness

Offer drug information and educate patients on proper medication usage

Collaborate with healthcare providers to ensure safe and effective medication management

Monitor patient progress and adjust medication regimens as necessary

Maintain accurate records and ensure compliance with all regulations Qualifications:

Active pharmacist license in California

Outpatient pharmacy experience preferred

Excellent communication and interpersonal skills

Ability to work independently and as part of a healthcare team
