Location Details

This position is situated in Montevideo, Minnesota—a charming community in Chippewa County along the Minnesota River, offering affordable living, excellent schools, and abundant recreational opportunities in the heart of western Minnesota.

Schedule

  • Monday-Friday: 8:30am – 5:30pm
  • Weekends: Closed

    Primary Responsibilities

  • Deliver comprehensive pharmaceutical care to long-term care facility residents
  • Verify prescription accuracy and appropriateness of medication therapy
  • Ensure strict compliance with state and federal pharmacy regulations
  • Maintain effective communication with healthcare providers regarding patient care
  • Conduct thorough medication therapy management services
  • Administer immunizations according to established protocols
  • Provide detailed patient counseling on proper medication use
  • Maintain accurate and complete patient records and documentation
  • Implement quality assurance measures and continual improvement initiatives
  • Manage pharmacy inventory to ensure optimal medication availability

    Required Qualifications

  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Current Minnesota pharmacist license in good standing or ability to obtain
  • Immunization certification
  • CPR certification
  • Medication Therapy Management (MTM) certification
  • Experience with long-term care pharmacy operations
  • Demonstrated clinical knowledge and analytical decision-making skills
  • Excellent communication and interpersonal abilities
  • Proven commitment to patient-centered pharmaceutical care
  • Proficiency with pharmacy management software and technology

    Additional Skills Required

  • Comprehensive knowledge of CMS regulations pertaining to long-term care
  • Experience with medication packaging systems and delivery protocols
  • Expertise in formulary management and drug utilization review
  • Ability to work independently with minimal supervision
  • Strong organizational and time management capabilities

  • Experience with medication synchronization programs
