Location Details

This position is located in Madison, Minnesota—a charming community in Lac qui Parle County with a strong sense of community and excellent quality of life.

Schedule

  • Monday-Friday: 8:30am – 5:30pm
  • Weekends: Closed

    Primary Responsibilities

  • Provide comprehensive pharmaceutical care to long-term care facility residents
  • Review and verify medication orders for accuracy and appropriateness
  • Dispense medications in accordance with state and federal regulations
  • Communicate effectively with healthcare providers regarding medication therapy
  • Conduct medication therapy management services
  • Administer immunizations according to protocol
  • Counsel patients on proper medication use and potential side effects
  • Maintain accurate patient records and documentation
  • Ensure compliance with all regulatory requirements
  • Participate in quality improvement initiatives

    Required Qualifications

  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Current Minnesota pharmacist license in good standing or ability to obtain
  • Immunization certification
  • CPR certification
  • Medication Therapy Management (MTM) certification
  • Experience with long-term care pharmacy operations
  • Strong clinical knowledge and decision-making skills
  • Excellent communication and interpersonal abilities
  • Demonstrated commitment to patient-centered care
  • Proficiency with pharmacy management software

    Additional Skills Required

  • Knowledge of CMS regulations pertaining to long-term care
  • Experience with medication packaging systems
  • Understanding of formulary management
  • Ability to work independently with minimal supervision
  • Strong organizational and time management capabilities
  • Commitment to continuing education and professional development
