Location Details

This position is located in Granite Falls, Minnesota—a picturesque community situated along the Minnesota River in Yellow Medicine County, offering an exceptional quality of life with abundant outdoor recreation opportunities.

Schedule

  • Monday-Friday: 8:30am – 5:30pm
  • Weekends: Closed

    Primary Responsibilities

  • Oversee comprehensive pharmaceutical care for long-term care facility residents
  • Ensure accuracy and appropriateness of all prescription orders
  • Maintain strict compliance with state and federal pharmacy regulations
  • Collaborate effectively with healthcare providers regarding medication therapy
  • Perform detailed medication therapy management services
  • Administer immunizations according to established protocols
  • Provide thorough patient counseling on medication use and potential side effects
  • Maintain precise patient records and documentation
  • Implement quality assurance measures and continuous improvement initiatives
  • Manage inventory and ensure optimal medication availability

    Required Qualifications

  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Current Minnesota pharmacist license in good standing or ability to obtain
  • Immunization certification
  • CPR certification
  • Medication Therapy Management (MTM) certification
  • Experience with long-term care pharmacy operations
  • Demonstrated clinical expertise and decision-making capabilities
  • Superior communication and interpersonal skills
  • Commitment to patient-centered pharmaceutical care
  • Proficiency with pharmacy management software systems

    Additional Skills Required

  • Comprehensive knowledge of CMS regulations regarding long-term care
  • Experience with medication packaging and delivery systems
  • Expertise in formulary management and drug utilization review
  • Ability to work independently with minimal supervision
  • Advanced organizational and time management capabilities

  • Experience with sterile and non-sterile compounding techniques
