Position Overview

As a Clinical Pharmacist in this advanced practice role, you will serve as an integral member of the healthcare team, working directly with providers to optimize medication therapy and improve patient outcomes within the correctional system. You will have the opportunity to practice at the top of your license, exercising your clinical judgment and expertise in a collaborative healthcare environment.

Location Details

This position is based in Wilsonville, Oregon, a charming suburban community located just south of Portland. Wilsonville offers an excellent quality of life with its proximity to the metropolitan amenities of Portland while maintaining a smaller community feel. The area boasts beautiful natural surroundings with easy access to outdoor recreation.

Schedule & Work Environment

  • Monday - Friday daytime hours
  • No weekend responsibilities
  • Occasional travel to nearby facilities (approximately one day per week)
  • Collaborative practice with internal providers
  • Patient-centered care model

    Advanced Practice Responsibilities

  • Conduct comprehensive patient assessments for acute and chronic disease states
  • Order and interpret drug therapy-related laboratory tests
  • Develop and implement medication therapy management plans
  • Provide direct patient care through referrals from providers
  • Collaborate with interdisciplinary healthcare teams
  • Participate in the evaluation and management of complex disease states
  • Make appropriate referrals to other healthcare providers
  • Lead quality improvement initiatives
  • Deliver education to both healthcare professionals and patients

    Required Qualifications

  • Doctor of Pharmacy degree from an accredited institution
  • Current Oregon pharmacist license in good standing
  • Minimum of 5 years of clinical pharmacy experience
  • Advanced practice experience in an institutional setting required
  • Strong clinical background in one or more of the following: hospital pharmacy, ambulatory care, federal prisons, or VA system
  • Clinical manager experience required
  • Demonstrated expertise in chronic disease state management
  • Exceptional communication and interpersonal skills
  • Strong ability to function independently and as part of a team
  • Experience with electronic health record systems
  • Knowledge of medication safety and quality improvement methodologies
