Job Title: Clinical Pharmacist
Term of Assignment: 13 Weeks

Overview:

We are seeking a skilled and versatile Clinical Pharmacist to provide comprehensive pharmacy services across multiple settings within the hospital.

This position includes a wide range of responsibilities, from sterile compounding to direct patient care, ensuring high-quality outcomes in medication management and patient support.

Key Responsibilities:

  • Order Verification: Review and verify medication orders to ensure accuracy and appropriateness.
  • Sterile Compounding & Verification: Prepare and oversee the preparation of sterile products, ensuring compliance with safety standards.
  • Clinic Inspections: Conduct inspections across various clinics to maintain compliance with safety and regulatory guidelines.
  • Rotational Pharmacy Work: Provide services across different hospital pharmacies, including:
  • Acute Care Pharmacy:
  • Verify orders, monitor drug therapies and patient labs.
  • Perform sterile product compounding and oversight.
  • Provide drug information to healthcare teams and counsel patients on discharge medications.
  • Adjust renal dosing per established protocols.
  • Primary Care Pharmacy:
  • Deliver drug information to nurses and physicians in primary care settings.
  • Conduct proactive chart reviews and pharmacist refill authorizations.
  • Provide anticoagulation services and administer adult and pediatric vaccinations.
  • Quyanna Care Center (Long-Term Care):
  • Conduct chart reviews, medication consultations, and prescription preparations, including medisets.
  • Outpatient Pharmacy:
  • Handle prescription entry, fill, and verification.
  • Counsel patients and administer vaccinations.
  • Village Health Services:
  • Optimize PickPoint systems, perform village clinic inspections, and support community health aides.

    Additional Responsibilities:

  • Collaborate with physicians on medication selection and drug therapy outcomes.
  • Participate in formulary reviews, monitor drug usage and cost-effectiveness, and ensure adherence to current practice standards.
  • Serve as a preceptor for pharmacy students and provide staff education.
  • Actively participate in P&T and other relevant committees.
  • Perform other duties as assigned.

    Requirements:

  • A clear and active pharmacist license in any U.S. state.
  • Inpatient pharmacy experience required.
  • Preferred Experience:
  • At least 2 years of inpatient or hospital pharmacy experience.
  • 1 year of post-graduate residency or equivalent pharmacist intern experience in a hospital setting.
